Polk County seeks vendors to provide radiology services for employees of the Polk County Board of County Commissioners, Polk County Sheriff''s Office, Polk County Property Appraiser, Polk County Tax Collector, Supervisor of Elections, and Clerk of Courts along with eligible family members participating in the County''s employee health plan. Vendor will provide radiology services to Health Plan Members as needed, including all radiology services described in Attachment B at specified rates. Health Plan Members with a prescription for radiology services will contact the approved vendor to schedule appointments and identify themselves as participants in Polk County Government''s employee health plan. Health Plan Members must complete the Radiology Referral form upon arrival for service. Any radiology services performed outside the provided list will be billed to the Health Plan Member''s medical insurance. Vendor will maintain a copy of the Radiology Referral form and submit it with invoices to the Risk Management Director.
